Give the account name and the code from the file you saved, and set a new password. Using the code replaces it: a new file is issued and the old code stops working.
If the file is gone there is nothing this page can do, because only a hash of the code is stored. What is left is to write to the operator from the address the account was registered with, who will ask you about the registration before issuing a new code:
